Summary:
This role is responsible for the provision of psychiatric case management services including but not limited to:
Meeting with patients/families to coordinate all facets of patient transition to community providers for longer term treatment.
# This position utilizes patient registries and data collection tools to support patients, collaborates with providers, completes transition plans to assist with coordinating community services and entitlement needs.
In context of care coordination, provides brief/short term supportive counselling to patients and families through the course of care.
Collects, tabulates data and communicates to the any potential risk management issues.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES:
Education # Training:
Bachelor#s degree in Social Work, Psychology or related field is required.
# Master#s degree preferred.
Work Orientation #
Experience:
## A minimum of two (2) years# experience in a mental health treatment setting is required.
Previous hospital and/or case management experience preferred.
Skills # Abilities:
Demonstrate ability to complete care coordination/case management functions and transition/discharge planning in accordance with state and federal guidelines.
Demonstrate competence in meeting with patient/families utilizing a person-centered approach.
Ability to provide psychiatric case management services including but not limited to crisis intervention, care coordination, entitlement and resource linkage.
# Demonstrate ability to (a) Communicate and collaborate professionally with both internal and external customers and community providers utilizing person-centered language and treatment approach; (b) Assess, adapt, and calmly respond to changing/or crisis environment; (c) Make decisions consistent with current policies, procedures, and ethical standards; (d) Support treatment plans in accordance with state and federal guidelines; (e) Demonstrate ability to implement and utilize evidence-based tracking tools and patient registries; (f) Interpret data, alert providers, and assist with treatment recommendations as appropriate; (g) Utilize computer and software systems; (h) Work effectively and efficiently in a fast-paced environment; (i) Obtain and track insurance authorizations/concurrent authorizations for Outpatient/Intensive Outpatient patients as necessary.

Estimated Salary: $20 to $28 per hour based on qualifications.
